With increasing numbers of us taking last minute world wide holidays, it is vital for us not to forget about our vaccination requirements.
Ideally we should all be seeking advice on our travel vaccines 6-8 weeks prior to our trip. However even if you are booking a trip on a tight schedule –you should consult with a travel clinic for up to date anti-malarial advice and travel risk assessment.

A small percentage of travellers will have a more severe form with bloody diarrhoea –known as dysentery. Most cases are self limiting and require the drinking of plenty of fluids and rest. The commonest pathogen worldwide is the bacteria E Coli.

Drink only bottled water if possible and avoid ice, uncooked meats and fruit and vegetables which have not been peeled.
Water purification tablets can be utilised, when bottled water is unavailable and avoid excess alcohol.
The use of antibiotics for diarrhoea prevention for the typical traveller is not usually required. This is because most cases of diarrhoea are mild and last only a few days and over use of antibiotics can lead to the build up of drug resistant strains.

The majority of travellers will simply need to ensure they drink plenty of fluids. People at higher risk due to old age, babies or those who have chronic medical conditions may require rehydration salts.
There is clinical evidence that the use of antibiotics should be considered for those who are suffering with more severe forms of diarrhoea. The course of antibiotics will lessen the duration and magnitude of the illness. Prevention however is always better than cure.

The commonest travel vaccine package usually includes- hepatitis A/typhoid and a diphtheria/tetanus/polio vaccine. Travel to Africa and South America often requires yellow fever vaccination.

This is a virus spread usually by food/water contaminated with human faeces.
It is the commonest disease which can be prevented by vaccination. The risks are highest when travelling to rural areas with poor sanitation. Symptoms can range from non specific tiredness, fever and joint pain to jaundice, enlarged liver and in rare cases liver failure.
The usual course of the illness is around 2-3 weeks and is managed with simple bed rest in most cases.
Vaccination is highly recommended for the vast majority of worldwide destinations.
It does take up to two weeks to develop immunity after vaccination, so book your travel clinical appointment in good time.

Typhoid-also known as enteric fever, is usually caused by the ingestion of contaminated food/water. It occurs often in poor areas of the world, where the standard of sanitation is low. There are 22 million cases of typhoid per year with 200,000 typhoid related deaths.
Common symptoms include fever, headache, joint aches early in the condition, followed by swelling of the abdomen and diarrhoea. The death rate is as high as 20% in untreated cases. Treatment with the appropriate combination of antibiotics is usually successful.
The vaccine is recommended for most travellers visiting areas in the developing world.
Immunisation against typhoid will last three years.

Yellow fever remains one of the most deadly viruses and the main risk areas are tropical Africa and South America. Like malaria, it is transmitted from the bite of mosquitoes. The estimated risk to unvaccinated travellers in endemic parts of West Africa is 100 per 100,000 of illness and 20 per 100,000 of death.
Initial symptoms are often vague and non specific including fever, malaise, nausea and headaches. This may then lead to a more severe haemorrhagic fever (a condition in which there is bleeding from bodily orifices and jaundice).
There is no specific treatment and therefore prevention is the key.
The yellow fever vaccination must be administered by a recognised yellow fever centre and an international certification of immunisation is issued. The yellow fever certificate may be required as evidence of vaccination when travelling in between countries.
The vaccine can take as long as ten days to provide protection and lasts for ten years.

Between 300-500 million people may become infected with malaria every year and 1 million die as a result of malaria infection per year.
It is caused by the parasite Plasmodium and spread through the bite of mosquitoes. It is common throughout many tropical areas of the world, where conditions for the breeding of the female mosquito Anopholes are ideal.
Clinical features of malaria are again non specific and we should be considering the disease as a possibility in any traveller returning from an at risk country with a fever.
Treatment will depend upon the type of malaria parasite and severity of illness.
There a number of different anti-malarials and guidance will depend on both location and length of stay.
Malarone is one of the most popular preventative medications being both highly effective against most strains and well tolerated. It only needs to be taken a day before travel, while on the trip and for one week after your return.
